Parents, Guardians, and Friends of Dobson Football –

 

Please consider volunteering to support Dobson Football. Some examples of volunteer efforts include:

  • Staffing the concession stand during Home games
  • Building and painting yard signs for the players
  • Ordering food from local restaurants, picking up food
  • Chaperoning or supervising players during fundraising events
  • Serving food to the players for their pre-game meals
  • Soliciting donations and sponsorships from local businesses
  • Creating flyers for parents, players, sponsors, etc.
  • Coordinating volunteers for fundraising events
  • Taking notes during meetings
  • Maintaining this website
  • And much more!

The 2015 Mountain Camp (aka Camp of Champions) is being held in Winslow Arizona. This is a mandatory camp for any student athlete to participate in Dobson Football during the 2015-16 Season.

Mesa Public Schools requires that all Student-Athletes submit a fully completed Athletic Participation packet and have a sports physical completed each year.

 

For Football players, completed packets are typically required before in Spring Football and are valid for the following Fall Season.

 

For incoming Freshman or new players who join after Spring Football, check with that team’s head coach for your packet deadline.

 

You MUST return your completed forms to the Dobson Athletics Department in the Administration building – Coaches cannot take these forms from you.

If you have problems access the above form, go to http://www.mesasports.org/sports_camps/ and look for Dobson Mustangs Sports Camps. In that section, you will find “Football Summer Strength Camp”.

IMPORTANT: Players are required to have completed a Sports Physical this year and turned in that paperwork before attending Football Camp. If you haven’t already done so, schedule an appointment with the doctor of your choice to complete the AIA Annual Preparticipation Physical Evaluation (pages 5, 6 and 7 of the packet below). Completed forms should be returned to Mrs. Cook in the DHS Athletic Department.

  • AIA Mild Tramatic Brain Injury (MTBI) / Concussion Annual Statement and Acknowledgement form (Page 4)

  • AIA 2014-2015 Annual Preparticipation Physical Evaluation (pages 5, 6 and 7). THESE 3 PAGES MUST SIGNED BY A PHYSICIAN AND RETURNED BEFORE PARTICIAPTION IN SPRING FOOTBALL IS ALLOWED!!!

  • Mesa Public Schools insurance/medical “blue card” (page 8)

  • Brainbook Concussion Awareness Statement and Acknowledgement Form (page 9)

  • Brainbook Concussion Awareness letter with instructions (page 10)
